GOING ONLINE: The International Association of Fire Chiefs wants firefighters to report such incidents on the Web site of the National Fire Fighter Near-Miss Reporting System.
ANONYMOUS REPORTING: The new Web site lets firefighters report such incidents anonymously and without fear of punishment in hopes that others can learn from them.
