DIXFIELD – SAD 21 is compiling a list of students for entry into kindergarten in the fall and is asking parents to provide the necessary information by Feb. 3.
To enter kindergarten in the fall, a child must be 5 years old on or before Oct. 15, 2004. Birth certificates will be required when registering.
Parents must provide a record of all preschool immunizations. Students entering school must have:
• Four doses of polio vaccine, at least one dose administered after the child’s first birthday.
• Five doses of D.P.T. vaccine for diphtheria, pertussis, and tetanus, or D.T. Vaccine.
• Two doses of measles/mumps/rubella vaccine administered after the first birthday.
The school nurse will be available to review immunization records and to provide information on immunization needed, dates of immunization clinic, and other information.
