FARMINGTON – Franklin Community Health Network has developed a page on the hospital’s Web site that tells people how to acquire copies of medical records from Franklin Memorial Hospital.

Patients can print a consent form, see examples of records that the hospital does not keep and find instructions on requesting records either in person, by mail or fax.

The page can be accessed at www.fchn.org/fmh/dept/MedicalRecords.

Requests for medical records must be in writing and become part of the person’s medical record.
