JAY – Kindergarten registration is scheduled for Thursday and Friday, April 2 and 3, at Jay Elementary School.

Registration and screening will be by appointment only. Call 897-5719 to schedule an appointment. A child must be age 5 on or before Oct. 15, 2009, for kindergarten registration. It is necessary to have the child’s original birth certificate and current immunization record at the time of registration.

The immunization record can be obtained from the child’s physician. A certified copy of the child’s birth certificate can be picked up at the town office where the child was born or at the town where the parents were residing at the time of the birth.

The cost in Jay for a birth certificate is $10 for the first copy and $5 for each additional copy.
