CONCORD, N.H. (AP) — Sens. Jeanne Shaheen of New Hampshire and Susan Collins of Maine are planning to introduce a measure this week to help Medicare patients avoid multiple hospital visitors and ensure a smooth transition to home or alternative care.

The senators say the Medicare Transition Care Act would bring down health care costs and improve care. It would increase support to patients, especially for ones with multiple, chronic conditions, as they move from the hospital to their new care setting, whether it’s a rehab center, home, or nursing home.

The measure has the support of such organizations as the AARP.
