LEWISTON — A $25,000 grant from Anthem Blue Cross and Blue Shield to Androscoggin Home Care & Hospice will fund in-home tele-health patient support as part of the health care agency’s Heart Failure Care Coordination Program.

The grant played a pivotal role in acquiring updated technology to support high-risk heart failure patients at home and help them avoid unnecessary re-hospitalization.

The Heart Failure Care Coordination Program was launched in 2009. With this latest investment, the program will serve 300 patients annually. It combines in-home monitoring with patient education that increases a client’s knowledge of their condition, understanding of self-care measures that decrease risk related to diet and medications, and ability to monitor symptoms that may require intervention by their physician.
