JAY — Jay Elementary School kindergarten registration is scheduled between 8 a.m. and 2 p.m. Monday, April 11. Registration and screening will be by appointment only. Call 897-5719 to schedule an appointment.

A child must be age 5 on or before Oct. 15, 2011. It is necessary to have the child’s original birth certificate and current immunization record at the time of registration.

The immunization record can be obtained from the child’s physician. A certified copy of the birth certificate may be obtained at the town office where the parents were residing at the time of the child’s birth. The cost in Jay for a birth certificate is $15 for the first copy and $6 for each additional copy.

Parents are encouraged to tell people who have a kindergarten-age student in Jay that they must register on April 11 for the fall of 2011.
