LEWISTON — Lewiston Public Schools will hold kindergarten registration and screening for the 2016-17 school year from 9 a.m. to 2:30 p.m. Friday, May 13.

Montello School, 407 East Ave.: 207-795-4150.

Registration must be done in the child’s district. Parent, medical and education informational forms will need to be completed. Required documentation will include an official birth certificate; an I-94 or green card if the child was not born in the U.S.; immunization records; and proof of residency (driver’s license with school-appropriate address, lease or rent agreement, utility bills or mortgage agreement).

The child must be present during registration and be 5 years old by Oct. 15.
