FARMINGTON — The National Committee for Quality Assurance has announced that Franklin Health Farmington Family Practice has received NCQA patient-centered medical home recognition.

It recognizes using evidence-based, patient-centered processes that focus on highly coordinated care and long?term, participative relationships. The practice received Level 3, the highest status.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. 

To earn recognition, which is valid for three years, Farmington Family Practice demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. 

Farmington Family Practice offers a full range of family medicine services from routine physicals to managing complex medical conditions such as heart disease and diabetes. Its providers include Jacqueline Caldwell, MD; Kendra Emery, DO; Calli Meader, PA-C; Kristine E. Sanden, DO; Linda Seabold, PA-C; and Brent Laflin, LCSW. It is in the Franklin Health Medical Arts Center on the Franklin Memorial Hospital campus.
