Wednesday, May 16, Phillips; call 639-2909, Joan Haggan.

Thursday, May 17, Kingfield; call 265-4132, Georgie Woodford.

Friday, May 18, Strong; call 684-3521, Mary Stinchfield.

Those who have a son or daughter who will be age 5 on or before Oct. 15 should call the school secretary to set up an appointment, giving the child’s name, birth date, parents’ names, mailing address and physical address.

On the day of screening, parents should bring their child, the child’s birth certificate and immunization records.
