When writing your information, please include the 5 w’s: Who, What, Where, When and Why.
Photos should be bright and clear. Identify the people in the photo from left to right and make sure the names are spelled correctly. If you want the photo returned, put your return address on the back. Electronic photos need to be in JPEG format and at least 180 dpi resolution, but not larger than 10MB.
